Disciplinary/Criminal History

All applicants are required to provide responses to questions about their disciplinary and criminal history. Students who respond “yes” to the disciplinary history question must provide information about the approximate date of each incident and the circumstances. Additionally, if the incident/s occurred at a college, the institution’s Dean of Students Office must also submit a letter of explanation. Student applications will be considered incomplete without either or both explanation/s. Students who, after reviewing the exceptions set forth below, respond “yes” to the criminal history question will be asked to provide information about the location and approximate date of each applicable incident and the circumstances. Students may also be required to produce additional information and be subject to a criminal record check. Applications which do not include a student’s explanation and/or all additional requested information will be considered incomplete and treated accordingly. All applicants with a disciplinary/criminal history will be reviewed for admission by the University Admissions Committee.  More information is available at ww.umass.edu/admissions/transferforms

(Note: You are not required to answer “yes” to the criminal history question if the criminal adjudication or conviction: (1) has been expunged, sealed, annulled, pardoned, destroyed, erased, impounded, or otherwise ordered by a court to be kept confidential; (2) was a first conviction for misdemeanor drunkenness, simple assault, speeding, minor traffic violations, or disturbance of the peace; or (3) any conviction of a misdemeanor where the conviction occurred more than five years prior to the date of this application, unless you were sentenced to imprisonment upon conviction of the misdemeanor, or you have been convicted of another criminal offense within the five-year period.)
