APPENDIX - APPLICATION INFORMATION

Agency Specific Questions

  1. Describe the most significant learning experience you have had during supervision or with a client.
  2. Describe your strength/limitations as a counselor/therapist. include some statement of those areas in which you expect to work/grow during your internship.
  3. Please discuss your orientation to counseling/therapy and what skills you see as necessary to promote a therapeutic relationship.
  4. Summarize your interest and experience in psychological assessment.
  5. Please state your level of interest in the area of learning disabilities and include any pertinent experience you may have had with this population.
  6. Statement of interest: Please discuss your specific goals for the internship and your reason for applying to this internship site.
  7. What are your plans after completing your degree?

In addition to your APPIC application form, the following materials must be received no later than November 20, 2006.

  1. Current vita.
  2. Official transcript for ALL your graduate work.
  3. Reference letters by three persons who have observed your academic and/or applied performance. Please ask that the reference letters address both strengths and weaknesses. At least one letter must be written by an individual who has supervised you in a counseling/psychotherapy field of experience.
