Common Application Transfer College Report or UMass Amherst Transfer Disciplinary Report
After reviewing application materials, transfer counselors may additionally require a Common Application Transfer College Report (available on the Common Application website) or a UMass Amherst Transfer Disciplinary Report  to determine if applicants have a disciplinary history at any of the FOUR-YEAR colleges or universities they have previously attended and, only if living on-campus, two year colleges. Without this information, their applications will be considered incomplete.
Criminal Record Check
If the nature of an applicant’s criminal history results in a positive response to the application criminal history question, then applicants are asked to provide (1) a statement which includes the location and approximate date of each incident and explains the circumstances, and (2) a signed Open Access Criminal Offender Record Information (CORI) Acknowledgement Form (if incident/s occurred in MA only) and/or Criminal Record Check report (if incident/s occurred in other states). UMass Amherst will use the CORI Acknowledgement Form to produce a CORI report. Report processes for other states will vary. Student applications will be considered incomplete without these documents.
Disciplinary Question Response Form
All applicants are required to provide responses to questions about their disciplinary and criminal history. When this section is left blank on application forms, those applications are considered incomplete. This form may be used to provide information about disciplinary and criminal history. Students who have not been enrolled in college course work for one or more semesters after high school graduation must provide an explanation about the reason for the gap/s in their education. This may take the form of a resume, a list of activities, or an essay.
Mid-Term Grades Form
After reviewing application materials, transfer counselors may additionally require mid-term grades to determine general admissibility or admissibility to a particular major. Without this information, their applications will be considered incomplete. Students may use the Transfer Mid-Term Report form available on the Common Application website, by going to the “My Colleges” tab, clicking on “Recommenders and FERPA"and printing the offline form from the Additional Forms section at the bottom of the page. Students may also choose to submit the UMass Amherst Mid-Term Grades form .
When students neglect to sign their application form, we have no verification that the information they provided is accurate. Without a signature, their applications will be considered incomplete. This form may be used to verify the accuracy of an application submitted without signature. Download the form .
International students studying in the U.S. on visas are required to submit both sponsor statement (promise to pay) and bank statement (verification of funds available) in order to complete their application. Students may use the UMass Amherst sponsor statement form.  Official bank statements are to be provided by financial institutions.
MassTransfer Alternate Major Form
MassTransfer guarantees admission only when you have completed all coursework with the required final GPA in a program approved for your desired major. Because you are completing your graduation requirements this semester and/or over the summer, we unable to determine if you will have the minimum final GPA required for your first–choice major at this time. Therefore, we will need you to select an alternate major. This form may be used for that purpose. Download the form. 
Transfer Alternate Major Form
University policy states that transfer students must declare a major in order to be admitted. Students who declare application major/s to which they are inadmissible must provide us with an alternate major to which they are admissible; otherwise, their applications will be considered incomplete. Tuition Residency Form
All applicants are required to provide information related to tuition residency as part of their application form. When that information is missing, applications are considered incomplete. This form may be used to submit tuition residency information. Download the form .
